Enrolment form - Easy reading printed version

The new feature of the printed enrollment form is very un- user friendly. The home address section, for example, is literally one word per line so this takes up a lot of wasted space on the page. Would appreciate a more visually appealing, easy to read, enrollment form. Thank you.
