Incident Reports - meeting the ACECQA's requirement

The Incident Reports are still missing fields based on Australian Children's Education & Care Quality Authority (ACECQA)’s template:
- Name of person who witnessed the incident/injury/ trauma/ illness
- Field of circumstances if child appeared to have been taken or removed from service or removed from service or was locked in/out
- Have any steps been taken to prevent or minimise this type of incident in the future?
- Notification to other agency (if applicable)
- Regulatory authority (if applicable)
- Please also added an area for noting the name of the first aider and follow-up areas after the incident, e.g. child’s diagnosis at hospital.
Besides, the Incident time to be captured at the time of creating the document. Is it possible to amend the time? I assume that educators may not always be able to complete the record as soon as it happens.

I have feedback from the department today and they have said Reg87 3c, requires a part of the incident form that says Medication administered Yes/NO, she said even though the form has a part that says action taken she said staff could over see this as its not as easy as yes/no
Could we please get this added to the incident form asap.

URGENT Non Compliance of Incident Froms

We had a spot check today and one of the things that they wanted to see was our incident forms. we gave them a demo form using Xplor Playground and a fake child. It has come back as not compliant with the national regulations, with particular reference to regs 62, 92, 162. This needs to be rectified urgently.

We all looking at going over to using the incident reports on playground and there is things missing on it, as ACECQA looks a little different, can this please be looked at

I agree with this feedback and also add the following:
A place to monitor temperature under illness.
A place for the RP or Director to acknowledge/sign before giving to the parent.

Once the report has been signed by educators and submitted you can no longer make changes to the incident form. My educators often need to add progress notes throughout the day as they monitor the child after the incident. New developments arise or further first aid related to the same injury. We should be able to add progress notes right up until the parent signs the form then it becomes finalized and you can no longer edit the document.

I agree this should be a priority. We currently do paper forms then upload to Xplor - what a hassle when the option is there to do them legally if adjustments were made

I agree with this. We don't use this function because we like to have management sign off on all incident reports to ensure that management are aware.
